Normally, we complete an automated ID verification using via Onfido which checks your name, DOB and/or address against various other records - such as the voting register, telephone database or credit agencies. This is so that we can cross-check the information you have provided with another source to eliminate fraudulent prescription requests and to ensure that we are prescribing to the correct person.
However, when there are discrepancies in DOB, last name or address, or when Onfido cannot locate secondary sources which confirm your name, DOB or address, the check will fail and we have to manually verify your identity.
If your ID verification has failed, please Submit a Request and attach a photo of your driver's licence or passport, and ensure you upload a copy to the consultation upon your Juniper licensed healthcare practitioner's request.
